About the Role

Key Responsibilities:

Medicines Management & Optimisation Support the processing of repeat prescriptions, ensuring safety, accuracy and compliance with practice protocols

Conduct medicines reconciliation following hospital discharges and other care transitions
Assist in structured medication reviews to improve patient adherence and safety Monitor prescribing trends and support cost-effective prescribing within the surgery Identify and address medication-related queries from patients and staff

Patient Support & Education Provide patients with advice on medication use, side effects and adherence Support long-term condition management by ensuring optimal medication use

Handle queries regarding prescriptions and medication changes

Prescribing & Administrative Support Assist with repeat prescribing processes, ensuring compliance with NHS policies Work with the clinical team to review and implement prescribing changes based on NHS guidelines

Support the practice in reducing medication waste and improving prescribing efficiency Safety & Compliance

Support compliance with local and national prescribing qualities (e.g. NICE, CQC, MHRA guidelines)

Assist with audits, medication safety reviews and reporting Ensure safe handling, storage and disposal of medicines in the practice

Teamwork & Collaboration Work alongside the clinical team to improve medicines management

Assist in training and supervision of other staff regarding prescription processes
Contribute to practice quality improvement initiatives related to medication safety.
